The Miller County Health Center is working with your child’s school to give the seasonal influenza vaccine (flu shot) to the students and staff. This vaccine will protect against all four influenza strains that are expected to circulate this year. We will be at the St. Elizabeth School on Monday, October 21, 2024.

To sign up your child and provide consent to be vaccinated, go to the following website: patients.vaxcare.com/registration

Your child will only get the vaccine with your consent on the website.

Select the name of your child’s school, complete the required information, and submit. Please note that your child has not been signed up unless you see a confirmation page upon submitting.

To complete the Payment Information section of the form, here is what you need to do: ď‚·

  • If your child has Medicaid, provide their Medicaid number in the area on the form indicating Medicaid. ď‚·

  • If your child is not Medicaid eligible, but has private insurance, provide the insurance information. ď‚·

  • If your child is not eligible for Medicaid and does not have private insurance, please indicate on the form in the “NO INSURANCE” area.

Regardless of your child’s insurance eligibility, the vaccine will be provided at NO cost to you. If you would happen to receive a bill for the vaccine, please do not send payment and call us at 573-369-2400.
